Form Filling Guide

The following are compulsory columns you have to fill.
•    pusat temu duga/ujian dipilih (interview centre/selected test),
•    nama pemohon (name of applicant),
•    tarikh lahir (date of birth),
•    negeri tempat lahir (state of birth),
•    jantina (gender),
•    taraf perkahwinan (marital status),
•    keturunan (race),
•    agama (religion),
•    kewarganegaraan (nationality),
•    tempat lahir ibu (mother’s place of birth),
•    tempat lahir bapa (father’s place of birth)
•    alamat surat-menyurat (correspondence address)
For the disabled, you must fill in the column under Kecacatan.
You must also fill up the Keputusan Bahasa Melayu Tingkatan 5 column for applicants in the degree, diploma, SRP and Bakat categories.
For those in the SPM category, you must fill in all the fields in the Form 5 results section.

For Kelulusan Pengajian Tinggi (Ijazah/Diploma/Sijil and telah diperolehi), you will have to fill in all the details of your application. Local University graduates will have to fill in the Universiti Tempatan section while overseas University graduates will have to fill in the contrary. If you are not sure what is your completion date, key in your convocation date.

For applicants in the Diploma/STP/STPM/HSC categories, you are required to fill in the Keputusan Peperiksaan Tingkatan 6 column while for Diploma holders, you will have to fill in the Kelulusan Pengajian Tinggi section.
You have to ensure that the information that you entered are true and accurate.
Once you have completed with entering all the information required, click on the button HANTAR. If there are no errors, a message that says ‘Permohonan anda telah didaftarkan’ will be displayed.
